Can a normal MRI rule out endometriosis?
A normal MRI cannot rule out endometriosis. MRI is most helpful for certain patterns like ovarian endometriomas or deeper disease, but superficial peritoneal endometriosis and fine adhesions often won’t show up on imaging—even when symptoms are significant.
If your symptoms persist despite a “normal” scan, the next step is a careful, endometriosis-focused review of your history and exam to decide what testing (if any) would actually change management. In some cases, repeating imaging with a different technique or reader can help, but when the clinical picture still strongly suggests endometriosis, diagnosis and treatment may require a targeted laparoscopy with an excision approach rather than relying on imaging alone.
