Can you have endometriosis with regular periods?

The short answer

Yes, you can have endometriosis even if your periods are regular. What often matters more is painful periods, pelvic pain around your cycle, pain with sex, bowel or bladder symptoms that worsen around your cycle, or infertility, even when bleeding is on schedule.

Yes, endometriosis can absolutely occur even if your periods are regular. A predictable cycle doesn’t rule it out: endometriosis is defined by tissue similar to the uterine lining growing outside the uterus, and symptoms don’t always match typical period patterns. Some people have severe pain with otherwise normal timing, while others have subtle symptoms (or none) despite significant disease.

What often matters more than cycle regularity is what happens around your cycle: painful periods, pelvic pain that flares before or during bleeding, pain with sex, bowel or bladder symptoms that worsen cyclically, or infertility, even when bleeding is on schedule.     Mild period cramping can be common, but endometriosis and adenomyosis are two common causes of intense period pain. Severe, worsening, or disabling pain, especially when it keeps you home from school or work, is a signal to look deeper.

  • Can you have endometriosis without pelvic pain?

    Yes, endometriosis can be present even without pelvic pain, and symptom severity doesn’t reliably reflect the extent of disease. It may show up as infertility, abnormal bleeding, pain with sex, bowel or bladder symptoms, or persistent bloating.

  • Can endometriosis cause heavy menstrual bleeding?

    Yes, endometriosis can be associated with heavy menstrual bleeding for some patients, but heavy flow is more strongly linked to uterine conditions, especially adenomyosis. Many people with endometriosis have a normal amount of bleeding but significant pain.
