How are endometriomas diagnosed—do I need MRI or surgery?
Most endometriomas can be diagnosed based on a high-quality transvaginal ultrasound, because they often have a recognizable appearance on imaging. In many cases, that’s enough to make a confident diagnosis and start planning next steps without doing surgery just to “confirm” it.
We typically use MRI when the ultrasound is unclear, when we need to better distinguish an endometrioma from other ovarian cysts, or when we’re mapping possible coexisting deep endometriosis before surgery. Surgery is generally pursued to treat symptoms, protect ovarian function, or address concerning features—not simply to prove the diagnosis.
