Is bleeding after sex a symptom of endometriosis?
The short answer
Some people with endometriosis notice spotting after sex, but bleeding after sex is not considered normal, even with endometriosis. It can also come from cervical changes, vaginal dryness, infections or polyps, so it deserves a thoughtful evaluation.

Bleeding after sex isn’t something we consider “normal,” even if you have suspected or confirmed endometriosis. Some people with endometriosis do notice post‑coital spotting, especially when sex triggers pelvic inflammation, uterine/cervical irritation, or deeper pain from adhesions and sensitivity in the upper vagina and cervix. That said, bleeding after sex can also come from other causes (like cervical changes, vaginal dryness/fragility, infections, polyps, or other uterine/cervical conditions), so it’s a clue that deserves a thoughtful workup rather than an assumption that it’s “just endometriosis.”
In our evaluation process, we look at the full pattern: whether bleeding happens with deep penetration or orgasm, whether it clusters around your cycle, and whether it comes with deep dyspareunia, pelvic pain after sex, bowel/bladder symptoms, or abnormal periods. We also consider conditions that can coexist with endometriosis (including adenomyosis) and use exam plus carefully selected imaging when helpful to clarify what’s driving the bleeding. If this is happening to you, explore our education on sex-related endometriosis pain and reach out to schedule a consultation. Our team can help connect the dots and build a plan aimed at lasting relief.

Questions people ask next
All symptoms questionsIs pain during sex a sign of endometriosis?
Yes, deep pain during sex or aching afterward can be a sign of endometriosis. Pain with sex is not specific to endometriosis: pelvic floor muscle dysfunction, vaginal dryness, fibroids, bladder pain conditions, and infections can cause similar symptoms.
Why do I get stabbing pelvic pain after sex?
Stabbing pelvic pain after sex can happen when intercourse or orgasm triggers sensitive pelvic tissues, nerves, or muscles. Endometriosis, sometimes adenomyosis, scar tissue, and overactive pelvic floor muscles can contribute to this pain.
What red flags are more urgent than endometriosis?
Sudden, severe pain with fever, repeated vomiting, fainting, a rigid abdomen or heavy bleeding is a red flag for something more urgent than endometriosis. Pelvic pain with a positive pregnancy test, chest pain, trouble breathing or a painful swollen leg are also urgent.
