Mild one-sided ovulation pain (often called mittelschmerz) can be normal, but pain that is severe, recurrent, or limits your daily life isn’t something we dismiss as “just ovulation.” When ovulation pain is intense month after month, we start looking for drivers like pelvic inflammation, scarring/adhesions, ovarian cysts such as endometriomas, and heightened nerve sensitivity—all of which can occur with endometriosis.


Ovulation pain is more concerning when it shows up alongside other patterns like painful periods, pain with sex, bowel or bladder symptoms, or difficulty conceiving.
