Deep Disease
What are the symptoms of bowel endometriosis?
The short answer
Bowel endometriosis can cause lower abdominal or pelvic pain, bloating, constipation and/or diarrhea, nausea, and pain with bowel movements. Symptoms may fluctuate with your menstrual cycle, and some people notice rectal bleeding, especially around menstruation.
Bowel endometriosis can cause digestive symptoms that often resemble IBS and may fluctuate with your menstrual cycle. Common patterns include lower abdominal or pelvic pain, significant bloating (“endo belly”), constipation and/or diarrhea, nausea and sometimes vomiting, and pain with bowel movements, often worse during periods.
Some people also notice rectal bleeding, especially around menstruation, along with broader endometriosis symptoms such as chronic pelvic pain, painful sex, urinary discomfort, fatigue, or a heavy feeling of pressure in the pelvis. Because bowel symptoms can come from inflammatory lesions near the bowel even without deep implants, imaging may help map disease but can’t reliably rule it out. If your symptoms sound cyclical or you’ve been treated for GI issues without clear answers, our team can help evaluate the full picture and discuss whether minimally invasive excision surgery is appropriate for both diagnosis and long-term relief.

Questions people ask next
All deep disease questionsIs it IBS or endometriosis affecting my gut?
Endometriosis gut symptoms often flare around your period or ovulation with pelvic pain, while IBS symptoms are more likely to track with food triggers or stress. The two can overlap, so IBS-like symptoms do not rule out endometriosis.
Can endometriosis cause IBS-like symptoms without bowel lesions?
Yes, endometriosis can cause IBS-like symptoms even without visible bowel lesions on imaging or at surgery. Pelvic inflammation, heightened nerve sensitivity, hormone-driven changes, and pelvic floor muscle overactivity can all contribute to bowel symptoms.
Is rectal bleeding during your period a sign of endometriosis?
Yes, rectal bleeding that predictably happens during your period can be a sign of bowel endometriosis. But it is not specific to endometriosis: hemorrhoids, fissures, inflammatory bowel disease, polyps and other GI conditions are other possible explanations.
IBS vs IBD vs endometriosis: how can I tell the difference?
IBS often causes fluctuating bloating and bowel changes, while IBD is more likely with blood in the stool, fevers or weight loss. Endometriosis can mimic both, with clues like painful bowel movements, deep pelvic pain and bowel changes that worsen before or during your period.
