What tests help explain severe pelvic pain with normal imaging?

The short answer

A detailed symptom history and pain-focused exam are often most useful when imaging is normal. Depending on symptoms, testing may assess pelvic floor problems, nerve-related pain or vascular causes; if surgery is part of your plan, biopsy can confirm endometriosis.

Normal ultrasound or MRI doesn’t rule out endometriosis or other conditions that can cause severe pelvic pain. When imaging is unrevealing, the most useful test is often a deeper, pattern-based evaluation: a detailed symptom timeline (cycle-related pain, bowel or bladder triggers, pain with sex, fatigue), your response to prior treatment, and an exam focused on mapping pain generators and identifying look-alike conditions.

Depending on your symptoms, we may recommend more targeted testing beyond standard pelvic imaging, such as expert reinterpretation of ultrasound/MRI, evaluation for pelvic floor dysfunction and dyssynergia, and assessment for pain drivers like central sensitization or small fiber neuropathy. In select cases, we also look for vascular causes that can mimic or worsen pelvic pain (like pelvic venous congestion or May-Thurner syndrome), hernias, or overlapping endocrine or immune issues that complicate the picture.

If surgery becomes part of your plan, tissue diagnosis can be key: excision with histology (biopsy) can confirm endometriosis and, in some situations, additional tissue-based profiling, such as hormone receptor activity or mast cell density, can help explain why symptoms are severe and guide next-step decisions.   • How can I get diagnosed with endometriosis if my ultrasound is normal?

    A normal ultrasound doesn’t rule out endometriosis; evaluation includes a detailed symptom history, targeted pelvic exam and, when appropriate, expert imaging. If surgery becomes part of the plan, testing removed tissue offers the most definitive confirmation.

  • How is neuropathic pelvic pain evaluated, and when is it urgent?

    Neuropathic pelvic pain is evaluated by reviewing symptoms and examining sensation and pelvic floor muscles; imaging may help. It is urgent with new leg weakness, foot drop, saddle-area numbness, fever with severe back pain, or sudden inability to pass urine or stool.

  • Why can imaging look normal when symptoms are severe?

    Imaging can look normal despite severe symptoms because many pain-generating conditions don’t reliably show up on ultrasound, CT, or MRI. Some disease can be small, superficial, or hard to see, yet still irritate nerves and surrounding tissues.

  • Can imaging tell endometriosis from IBS or bladder pain?

    Imaging can sometimes provide helpful clues, but it can’t reliably tell endometriosis apart from IBS or bladder pain on its own. Ultrasound or MRI can be normal despite significant symptoms, so results are interpreted alongside your symptom history and a targeted pelvic exam.
