Will egg retrieval worsen endometriosis?

The short answer

For many patients, egg retrieval and IVF stimulation do not appear to dramatically accelerate endometriosis in the short term. Temporary pain flares are common, and limited data suggest stimulation may contribute to progression in some cases of deep infiltrating endometriosis.

For many patients, egg retrieval and the IVF stimulation that comes with it do not appear to dramatically accelerate endometriosis in the short term, even though estrogen levels can rise significantly during stimulation. That said, it’s common to feel worse temporarily: ovarian enlargement, inflammation, and pelvic pressure can trigger pain flares, and symptoms may be more noticeable once you’ve been off hormonal suppression to pursue pregnancy.

The main caveat we take seriously concerns deep infiltrating endometriosis and patients with significant baseline symptoms, where limited data suggest stimulation may contribute to progression in some cases. In practice, the decision is often a balance: IVF may help you move toward pregnancy sooner and reduce the total time you’re off symptom-controlling therapy, but the plan should be individualized around your disease pattern, endometriomas, prior surgeries, and quality-of-life goals. If you’re weighing egg retrieval and worried about symptom escalation, our team can help you think through timing, coordination with fertility care, and whether treating endometriosis surgically first (or after retrieval) makes the most sense for your body and timeline.

Questions people ask next

All fertility questions
  • Do endometriomas affect fertility or ovarian reserve?

Yes, endometriomas can affect fertility and may be associated with lower ovarian reserve in some patients. Surgery, especially repeat cyst surgery, can also affect ovarian reserve because healthy ovarian tissue can be removed or its blood supply altered.

  • Can endometriosis worsen even if pain gets better?

Yes, endometriosis can worsen even if pain gets better, because pain severity doesn’t reliably track with the disease’s extent or activity. Symptoms can improve without necessarily meaning lesions, adhesions, or organ involvement have stopped progressing.

  • Does endometriosis affect egg quality or implantation?

Endometriosis can affect both egg quality and implantation. In the ovaries, especially with ovarian endometriomas, inflammation may affect how eggs mature, while changes in the uterine lining may make it less receptive to implantation.

  • Can endometriosis surgery improve my chances of conceiving?

Excision surgery can improve the chances of conceiving for some patients with endometriosis, particularly when removing superficial disease helps restore more normal pelvic anatomy. Outcomes vary, and surgery on the ovaries can affect ovarian reserve.
