Yes, an ovarian cystectomy can improve fertility in some situations—especially when a cyst is disrupting ovulation, distorting pelvic anatomy, or driving inflammation, as we often see with endometriomas. By restoring normal ovarian function and pelvic anatomy, surgery may remove a barrier to conception and reduce pain that can interfere with timing and intercourse.


At the same time, any ovarian surgery carries a real tradeoff: it can reduce ovarian reserve depending on cyst size, whether it’s recurrent, and how much healthy ovarian tissue is affected during removal.
