Yes—adenomyosis can cause deep pain during intercourse and cramping or aching afterward. When endometrial-like tissue grows within the uterine muscle, the uterus can become enlarged, inflamed, and very tender, which may make deep penetration feel painful and can trigger lingering cramps.


Because adenomyosis commonly overlaps with endometriosis, pain with sex isn’t always from one condition alone.
