Can adenomyosis cause diarrhea?

The short answer

Yes, adenomyosis can be associated with diarrhea for some patients, especially around their period. It stays within the uterine muscle, but inflammation and cramping can trigger loose stools; endometriosis or bowel sensitivity may also contribute.

Yes, adenomyosis can be associated with diarrhea for some patients, especially around the menstrual cycle. Even though adenomyosis stays within the uterine muscle (it doesn’t spread onto the bowel), the inflammation, cramping, and pelvic nerve cross-talk it creates can “spill over” and trigger IBS-like bowel urgency or loose stools.

That said, diarrhea isn’t specific to adenomyosis, and bowel symptoms are often a clue that more than one issue may be contributing, most commonly endometriosis (which can involve tissue outside the uterus) and/or bowel sensitivity patterns that overlap with pelvic pain conditions.     Period diarrhea tends to worsen before or early in your period, while IBS symptoms often fluctuate with stress, sleep disruption, and certain foods. When cycle-linked diarrhea comes with significant pelvic pain, we consider endometriosis as well as IBS.

  • Does diarrhea mean I have bowel endometriosis?

    Not necessarily: diarrhea can occur with endometriosis due to hormone-driven changes in gut movement and pelvic inflammation, even without bowel wall involvement. Pain with bowel movements, rectal pain or pressure, bleeding or cycle-related flares raise more suspicion.

  • Can endometriosis cause diarrhea only during your period?

    Yes, endometriosis can be associated with diarrhea that happens only during your period. Endometriosis-related inflammation can make period-driven bowel changes more intense, even when endometriosis is not growing into the bowel wall.

  • Can adenomyosis cause pain with bowel movements?

    Yes, adenomyosis can cause pain with bowel movements. Because the rectum sits directly behind the uterus, uterine cramping and pressure can be felt as rectal pain even when the bowel itself is not affected. Painful bowel movements can also have overlapping causes.
