How often does imaging miss endometriosis?

The short answer

How often imaging misses endometriosis depends on the disease type, location, scan protocol and the experience of those performing and reading the scan. A normal ultrasound or MRI does not rule it out, especially superficial disease or subtle lesions.

Imaging misses endometriosis more often than most patients are led to believe. Ultrasound and MRI can be very helpful for suggesting endometriosis and mapping certain patterns of disease (like ovarian endometriomas or some forms of deep disease), but a normal scan does not rule out endometriosis, especially superficial peritoneal disease, subtle lesions, or disease hidden in locations that are difficult to visualize.

How often imaging misses it depends on the type of endometriosis you have, where it is, the scan protocol, and, crucially, the experience of the person performing and interpreting the study. It’s also common for one modality to detect something the other doesn’t, because each has different blind spots (for example, bowel gas can limit MRI in some situations, while ultrasound may miss certain deep sites). That’s why our diagnostic process doesn’t hinge on imaging alone.   • Should I trust normal imaging if symptoms persist?

    A normal ultrasound or MRI can be reassuring, but it doesn’t automatically explain away ongoing symptoms. Endometriosis, and sometimes adenomyosis, can still be present, so your symptom history remains the primary guide when symptoms persist.

  • How accurate is MRI vs expert transvaginal ultrasound for endometriosis?

    MRI and expert transvaginal ultrasound tend to have similarly high accuracy for deep infiltrating endometriosis and ovarian endometriomas. Both can miss superficial peritoneal endometriosis, so normal imaging does not necessarily rule out endometriosis.

  • How can I get diagnosed with endometriosis if my ultrasound is normal?

    A normal ultrasound doesn’t rule out endometriosis; evaluation includes a detailed symptom history, targeted pelvic exam and, when appropriate, expert imaging. If surgery becomes part of the plan, testing removed tissue offers the most definitive confirmation.

  • When is imaging most helpful for endometriosis?

    Imaging is most helpful when clarifying what may be driving your symptoms before treatment, or when we suspect deeper disease that may involve the bowel or bladder. It can help guide surgical planning, but it can’t rule out endometriosis.
