Standard pelvic ultrasound vs endometriosis-focused scan: what’s different?
A standard pelvic ultrasound is designed to survey the uterus and ovaries and can identify things like fibroids, ovarian cysts, or endometriomas—but it may not capture subtler signs of endometriosis, especially deeper disease or scarring that limits organ movement. Because it’s often a general “look around,” important details like tethering or small deep nodules can be missed even when symptoms are significant.
An endometriosis-focused scan is performed with endometriosis mapping in mind, typically using detailed transvaginal imaging plus a dynamic exam that evaluates how pelvic organs move against each other (often called the sliding sign). The sonographer may also correlate imaging with site-specific tenderness and obtain targeted views of common deep locations; in some settings, transrectal or transperineal approaches are used to visualize hard-to-see areas. This type of scan can provide a clearer roadmap for treatment discussions and, when surgery is considered, help with surgical planning—our team can review your imaging and symptoms together to decide what the next best step is.
