Sometimes, surgery is worth considering before pregnancy when adenomyosis is focal (a localized adenomyoma) and it’s clearly contributing to heavy bleeding, significant pain, or distortion of the uterine cavity—factors that can interfere with implantation and pregnancy. In carefully selected cases, removing a focal lesion may improve symptoms and may support fertility goals.


Diffuse adenomyosis is more challenging to address surgically because it involves a broader area of the uterine muscle, and any uterine surgery brings important considerations about healing, uterine strength in pregnancy, and delivery planning.
