How is deep infiltrating endometriosis diagnosed without surgery?

Specialized imaging is the main way we evaluate deep infiltrating endometriosis without surgery. An expert-performed transvaginal ultrasound (and sometimes transrectal ultrasound, depending on symptoms and anatomy) can identify and map deep disease by assessing how tissues move, where areas are fixed, and whether nearby structures appear tethered or involved.


Pelvic MRI can add detail about the depth and extent of suspected lesions and their relationship to organs like the bowel, bladder, and uterosacral ligaments. When imaging is done and interpreted by an experienced team, it helps us plan the most appropriate next steps and reduces “surprises” if surgery becomes part of your treatment plan.
