Pelvic MRI is most helpful when we suspect deep endometriosis involving structures like the bowel, bladder, ureters, or pelvic nerves, because it can help map disease patterns beyond what a routine exam can show. We also commonly use MRI when a high-quality ultrasound doesn’t fully explain symptoms or when imaging needs to clarify what’s driving pain, bowel, or urinary issues.


MRI can be especially valuable for preoperative planning, since understanding the likely locations and depth of disease helps us plan the safest, most complete approach. It may also be considered when symptoms persist despite prior treatment and knowing the extent of disease could meaningfully change next steps.
