Is CA-125 useful for diagnosing endometriosis?
The short answer
CA-125 is not a reliable way to diagnose endometriosis. It can be normal when endometriosis is present or high for other reasons, including other pelvic conditions and benign inflammation, so results usually don’t clarify whether endometriosis is causing your symptoms.
CA-125 can be elevated in some people with endometriosis, but it’s not a reliable way to diagnose it. The main limitation is that CA-125 is nonspecific: it can be normal even when endometriosis is present, and it can be high for other reasons (including other pelvic conditions and benign inflammation). Because of that, a normal or high CA-125 result usually doesn’t answer the questions patients need answered: is endometriosis driving your symptoms, where is it, and what else might be contributing?

Is there a reliable blood test for endometriosis?
No, there is currently no FDA-cleared blood test that can reliably diagnose or rule out endometriosis. Tests such as CA-125 and newer panels can be affected by the menstrual cycle and other conditions, so they are not specific enough to confirm it on their own.
How close is a non-invasive test for endometriosis?
Non-invasive tests for endometriosis are getting closer, but none have consistently met the accuracy needed to diagnose it reliably on their own. In the near term, biomarkers are expected to support decisions rather than replace expert imaging or laparoscopy.
